2026-27 Distribution & Collection
- Mon 9/21: Students to receive physical cards in Homeroom
- Students with a Non-Reporting Homeroom should pick up their card from Ms. Taylor in Rm 620
- Mon 9/28 (or before): Students to return cards to Homeroom teacher
- Mon 10/5: Final date to return cards to Homeroom teacher
- Tus 10/6: Front Office Staff begins reminder campaign via Blackboard voice/text/email
- Mon 9/21: Students to receive physical cards in Homeroom

2026-27 Help Guide (Slideshow)
- Cards are preprinted with student demographic information and address
- The entire form should be filled out in blue or black ink, completely in one color. Pen only, no pencil.
- Corrections should be made using a single line-through and print the correct information. Whiteout should not be used, or the document will be returned for corrections.
- Address changes must be accompanied by a valid Proof of Residence to be changed in the Student Information System (Infinite Campus)
- Document must be signed by a Parent or Guardian. Students over the age of 18 are not permitted to sign the document for themselves.
